An investigation has been completed after the licensee called in to self report an incident that occurred on the second day of school. Records were reviewed, staff were interviewed, and driving observation was made by the Inspector about a transportation allegation. Standard #: 22VAC40-185-40-E Description: Based on interviews and record review, the center failed to ensure that the center's services when transporting children to school was maintained in compliance on September 7, 2016. Center's policy requires that the staff drop children off when the school is open but staff dropped children off at school 10 minutes before the school opened. Plan of Correction: Bus routes were revised to ensure adequate travel time. Staff responsible for bus routes were retrained on policies and procedures regarding school age transportation.
Standard #: 22VAC40-185-340-A Description: The center staff failed to ensure the care and safety of 6 school age children when they dropped them off at school before the doors opened and the children remained outside for up to 10 minutes until staff took them inside. Evidence: 1. The staff were interviewed and acknowledged that a substitute driver had to be used the second day of school due to staffing shortage. The staff who transported the children stated she left the building around 835 and had to load two groups of school age children onto the bus because she had two schools to drop children off. She stated that it took time to get to the school and was worried because she thought the second group would be late. She stated she pulled up to the bus drop off area and saw adults standing outside and said she was not certain what to do because she was a sub. She said when she saw an adult walking towards the bus, she thought they were staff and opened the door and checked off the children. 2. The staff stated there was unusual issues occurring on that morning because two staff were late arriving, another staff was out, and they were trying to get the "Bus Runs" taken care of. Two staff confirmed the group exited the building with the staff member at around 830. One staff said she clocked in early because they were short of staff was signing in when the children were leaving and the Inspector was able to view her clock in time that read 8:32. 3. Upon driving the route to the school twice and both times were during morning traffic, it was determined that it took approximately 10 minutes to make it over to the school which would have put it around 845 AM. The school doors open at 855. Plan of Correction: Bus Routes were revised to ensure adequate travel time. Staff responsible for bus routes were retrained on policies and procedures regarding school age transportation. Additional staff have been certified to provide added coverage.
