An announced investigation was initiated today at 1pm in response to a self-reported incident of a child not being within sight and sound supervision on October 6, 2016. Based on written statements, it was determined that child #1 walked out of a classroom, into the hallway and up the stairway to her classroom unsupervised. The evidence gathered supports the claim of the child being unsupervised, so this has been determined to be "valid". See Violation Notice for more information. Description: Based on written statements, it was determined that a child was not within actual sight and sound supervision at all times while in care at the center. Evidence 1. Staff #1 and #2 stated that at approximately 5:30 on October 6th, they were preparing the children to line up in the playground area to go back to their classroom. They had entered the building from the playground and were in the empty Toddler #3 classroom when one of the teachers stopped to talk to a parent who had arrived to pick up their child. While the children were waiting for teacher #1 to finish her conversation, the children began to play with the equipment/toys in the classroom. While this was happening, child #1 walked out of this classroom, into the hallway and up the stairway to her classroom. The director saw the group of children in the classroom and noticed that child #1 was not with the group. The director then went upstairs and saw the child standing next to her classroom, which is located next to the stairway. It was estimated that the child was out of sight and sound supervision for approximately 2 minutes.

Plan of Correction: The director counselled the teachers that evening. They were instructed that if cutting through a classroom, that they keep all doors closed until they were ready to transition. The director also reviewed with all staff the sight and sound supervision policy (including name-to-face procedures) and reminded them to only complete one transition at a time with each group of children.
