Technical Assistance:
1. Documentation of annual file reviews for children's emergency contacts needs to include month/year for verification. 2. All sections of the enrollment documents should be completed with no blank spaces left. "N/A" is acceptable if the information does not apply to the child.

Comments:
Conducted an unannounced monitoring inspection 1:00-2:20pm. Observed 49 children + 6 direct-care staff. Ratios and supervision were in compliance. Children were napping during the inspection. Classrooms were found to be clean, safeguarded appropriately, and well supplied with toys and equipment for the children. The outdoor playground was found to be hazard free. According to the posted menu, lunch served included: fish nuggets, ketchup, mashed potatoes, corn, and milk. Medication management was in compliance. Required documents were posted. Standard #: 22VAC40-185-160-A
Description: Based on interview conducted, it was determined that an independent contractor has not submitted a negative TB screening. Evidence: The Director stated that the staff working for the dance contract program have no documentation of a negative TB screening.

Plan of Correction: Documentation has been received.

Standard #: 22VAC40-191-40-D-3-A
Description: Based on interview conducted, it was determined that an independent contractor has not obtained background checks. Evidence: The Director stated that the staff working for the dance contract program have not provided a Sworn Disclosure Statement, Criminal Record Check, or Central Registry Check.

Plan of Correction: Documentation has been received.

Standard #: 22VAC40-191-60-C-1
Description: Based on records reviewed, it was determined that 2 of 2 staff records checked did not include a completed criminal record check (CRC) within first 30 days of employment. Evidence: 1. Staff #7 - Date of hire was 8/15/16 and there was no CRC on-file. 2. Staff #8 - Date of hire was 8/9/16, and there was no CRC on-file.

Plan of Correction: Documentation has been received.

Standard #: 22VAC40-191-60-C-2
Description: Based on records reviewed, it was determined that 1 of 2 staff records checked did not include a completed central registry check (CPS) within the first 30 days of employment. Evidence: Staff #8 - Date of hire was 8/9/16, and there was no CPS on-file.
