Technical Assistance:
1. Reviewed documentation requirements for written injury reports.

Comments:
Conducted an unannounced monitoring inspection 3:00-3:50pm. Observed 135 children + 11 direct-care staff. Ratios and supervision were in compliance. Children were observed arriving to the program, washing hands, eating snack, doing homework, and having indoor free-play. Snack served: fruit + grain bars, mozzarella cheese sticks, and water. Staff were actively engaged with the children and assisting them with tasks as needed. Activity areas were found to be clean and sufficiently supplied with toys and equipment for the children. Medication management was in compliance. Required documents were posted. Record keeping was in order. Questions about this inspection may be directed to: pamela.sneed@dss.virginia.gov

Violations:
Standard #: 22VAC40-185-260-A
Description: Based on records reviewed, it was determined that there was no documentation that an annual fire inspection had been completed. Evidence: The previous fire inspection expired 9/1/16.

Plan of Correction: The Director of Operations will obtain an updated permit from ACPS Head Quarters.

Standard #: 22VAC40-185-280-B
Description: Based on observations made, it was determined that hazardous substances were not kept in a locked place and were accessible to the children. Evidence: A container of Clorox cleaning wipes were sitting on a desk in one activity area, and a custodial cart with multiple cleaning products was sitting in a hallway.

Plan of Correction: The Site director will meet with the school custodial staff to ensure that School staff have a clear understanding of items that must be locked and inaccessible. A CK manager will also routinely monitor activity areas to ensure school personnel have stored hazardous substances in a locked place.

Standard #: 22VAC40-185-400-B
Description: Based on observations made, it was determined that behavioral guidance delivered to a child was not constructive in nature, or appropriate. Evidence: A staff person was observed by the inspector speaking to a child about walking to get his snack and stated, "...use your walking feet...act like you have some sense..."

Plan of Correction: The Site director will review proper behavioral guidance techniques with the Staff member.
