Comments:
The inspector conducted an unannounced monitoring inspection from 10:30-12:30 p.m. There were 14 children in care with two staff supervising at all times. Children were observed being read to, lining up for the playground, playing inside and outside, at lunch, and washing hands after lunch. No medications are being administered at this time. One bus that is only used for field trips was inspected. Three children's records were reviewed. Two staff records were reviewed at the main office on 8/8/2016. The violation from the last inspection had been corrected. Violations are identified on the violation notice. Please call your inspector at 276-698-9981 with any questions.

Violations:
Standard #: 22VAC40-185-40-D
Description: Based on observation, the center did not have posted the license in a conspicuous place to the public. Evidence: 1. The inspector observed the license behind papers on a clipboard on the wall.

Plan of Correction: The director moved the license so that it was visible and said that she would post it separately on it's own.

Standard #: 22VAC40-185-70-A
Description: Based on observation and interview, the center did not have available at the center a telephone number for staff that would be notified in case of emergency or the required health information. Evidence: 1. Staff was not able to locate the required staff information.

Plan of Correction: The director said she would contact the main office for the information and make sure staff knew where it was kept.
