Technical Assistance:
Today the children were cared for in the elementary school gym rather than the auditorium. Due to the distance between the gym and the auditorium it is recommended that the center create a written plan to ensure the transport of all materials needed for the licensed program are transported from the auditorium to the gym whenever the gym is used for the entire before or after school program day.

Comments:
An unannounced monitoring inspection was conducted today from 5:15pm through 6pm. There were 7 children in care with three staff. Staff records are stored at a central location and will be reviewed at a later date. Children's records were reviewed. Children were observed in play activities with staff. A violation in the area of physical plant was observed and may be reviewed on the violation notice.

Standard #: 22VAC40-185-60-A
Description: Based on record review and interview the center failed to maintain a record for each enrolled child containing all required information. Evidence: 1 - The record for child 1 has a documented date of enrollment as 9/20/16. However, the record does not contain documentation of agreement to communicable disease notification, work information for parents, authorized pick-up information, authorization for emergency medical care. 2 - Staff 1 confirmed that the information was not present in the child's record

Plan of Correction: Center management will request and obtain the information from the parent.

Standard #: 22VAC40-185-320-B
Description: Based on observation and interview the center failed to ensure bathrooms are equipped with toilet paper within reach of children. Evidence: 1 - The boys bathroom does not have toilet paper. There is only one toilet stall in the boys bathroom. 2 - Staff present confirmed that there is not any toilet paper in the boys bathroom.

Plan of Correction: Going forward center staff will check the rest rooms used by children in care prior to children arriving to ensure that there is toilet paper available in each stall.
