Timing of background checks for new hires bringing original background checks from another facility was discussed.
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 1:45 PM and departed at 3:15 PM. Nap time and preparation for departure were observed. Medication administration and injury records were reviewed. A sample size of five children's records and four staff records were reviewed. At the closure of the inspection, an exit meeting was conducted with the program director during which plans of correction for the violations cited were obtained. Standard #: 22VAC40-185-60-A Description: Based upon review of children's records and staff interview, the facility has not ensured that each child's record includes the names, addresses and telephone numbers of at least two designated persons to be contacted in an emergency if a parent cannot be contacted. Evidence: 1. The record provided for child 3 did not include any designated persons to be contacted in an emergency if a parent could not be contacted. 2. The record provided for child 4 did not include the street address for one of the designated persons to be contacted in an emergency. 3. Staff 6 was unable to provide the emergency contacts for the above children. Plan of Correction: The facility responded that all children's records will be checked to ensure that all required information has been completed.
Standard #: 22VAC40-185-510-C Description: Based upon review of children's medications, the facility has not ensured that the facility's procedures for administering medication include methods to prevent the use of outdated medic. Evidence: There was a prescription package of albuterol for emergency administration to child 6 that expired on 10/18/2016. Plan of Correction: The facility responded that the medication will be returned to the parent. All medication will be checked and expiration dates will be noted to ensure parents are given adequate time to replace the medicine in the future.
Standard #: 22VAC40-191-60-B Description: Based upon review of the record for one new employee, he facility has not ensured that a new employee has completed a sworn statement or affirmation prior to employment. Evidence: 1. Staff 1 was hired on 09/26/2016. There was no signed sworn statement or affirmation in the record provided for staff 1. 2. Staff 5 was unable to provide a signed sworn statement or affirmation for staff 1. Plan of Correction: The facility stated that the employee will be required to complete and sign a sworn statement. In the future, this form will be required prior to a decision regarding employment.
