An unannounced inspection was initiated in the facility on October 5, 2016 regarding supervision. The inspection included observations, a review of center documentation and interviews with staff members and parties involved. Based on information obtained during the inspection the preponderance of evidence does support the allegation regarding supervision therefore the complaint is determined to be VALID. Areas of non compliance are noted on the violation notice. Please complete the columns for description of action to be taken and date to be corrected for each violation cited on the violation notice, and then return a signed copy to the licensing office within 10 calendar days of receipt. Description: Based on staff interviews and documentation reviewed, it was determined staff did not ensure a child's care and protection. Evidence: 1- During interviews and documentation Staff #1 and staff #3 confirmed on August 18, 2016, staff #3 fed child #4 from child #3's bottle containing 1/2 breast milk and 1/2 formula . According to staff , all the bottles were labeled with the child's name and dated and color coded for breast milk and formula. 2- During the interviews and documentation, Staff #1, staff #2 and staff #3 confirmed on September 26, 2016 child #1 was fed from child #2's bottle containing breast milk . Staff #1 went into the infant room to give staff #2 a break. Staff #1 checked which child needed to be fed. Staff #1 was looking for child #2's bottle and asked staff #3 which bottle did she feed child #1. She looked at the label with the name and date and realized it was child #2's bottle. According to staff , all the bottles were labeled with the child's name and dated and color coded for breast milk and formula. Staff #2 admitted to feeding child#1 the full bottle of breast milk.
