Technical Assistance:
The licensing inspector discussed the mandated reporting laws in the Code of Virginia with the owner and director.

Comments:
An unannounced inspection was conducted to investigate a complaint related to the supervision of children at the center. The inspection was conducted with the owner, director, and member of the local agency beginning at 9:30 AM and ending at 10:45 AM. Three staff interviews were conducted. One child's record and one staff record were reviewed and determined to be in compliance. The video recording of the incident was reviewed. A classroom observation was conducted. It was determined that the complaint was valid and corrective action plan had been developed by the center prior to the investigation. The owner and director spoke with the licensing inspector about alternative security measures to implement in order to ensure the safety of the children at the center. Description: Based on interview of three center staff members and review of the video recording from the facility's surveillance camera it was determined that not all children under the age of 10 years old were within actual sight and sound supervision of staff. Evidence: On October 19, 2016 at 10 AM child A was able to exit the center through the front double doors. The child crossed the parking lot and went into the street in front of the center. According to the time stamp on the video recording the child was exited the center at 10:01 AM and was returned to the facility at 10:03 AM. Child A was returned to the center by a member of the community.

Plan of Correction: The center developed corrective action plan prior to the inspection. Teachers will walk behind the students during transitions. The center updated the child information sheet requesting information about students that have wondering or hiding behaviors. Children identified with wondering or hiding tendencies will hold hands with the teacher during transitions. The center is exploring other security measures to ensure the safety of the children at the center.
