Comments:
An unannounced complaint inspection was initiated in the center on October 18, 2016 and completed it on October 28, 2016 in response to a complaint that was received regarding the area of supervision, staff qualifications and training and special care provisions and emergencies. Related observations were made, documentation was reviewed and interviews were conducted with center staff and with other parties involved. Based on the information obtained during the inspection there was not a preponderance of evidence that supports the allegation related to special care provisions and emergencies, however there was a preponderance of evidence that supports the allegation related to supervision and staff qualifications and training and therefore the complaint is noted as valid. Description: Based on a record review and staff interviews, the licensee did not ensure all the required information for staff records was kept for each staff person. Evidence: During the inspection on October 18, 2016, the record for staff #1 only inducted 1 of the 2 required references before employment or volunteering.

Plan of Correction: The assistant center director will make sure everyone has their references.

Description: Based on a record review and staff interviews, the licensee did not ensure staff received the required training by the end of their first day of assuming job responsibilities. Evidence: During the inspection on October 18, 2016, the center director confirmed the record for staff #1 did not include information indicating the staff had received training in the following areas; 1) Job responsibilities and to whom they report; 2) The policies and procedures listed in the standards as they relate to the staff member's responsibilities 3) The center's playground safety procedures unless the staff member will have no responsibility for playground activities or equipment 4) Recognizing child abuse and neglect and the legal requirements for reporting suspected child abuse as required by the Code of Virginia 5) Confidential treatment of personal information about children in care and their families;

Description: Based on a record review and staff interviews, the licensee did not ensure that all staff had by the end of the first day of supervising children all the required information in writing. Evidence: During the inspection on October 18, 2016, the center director confirmed that the record for staff #1 did not include information indicating the staff had received in writing the following information 1) Procedures for supervising a child who may arrive after scheduled classes or activities including field trips have begun 2) Procedures to confirm absence of a child when the child is scheduled to arrive from another program or from an agency responsible for transporting the child to the center 3) Procedures for identifying where attending children are at all times, including procedures to ensure that all children are accounted for before leaving a field trip site and upon return to the center 4) Procedures for action in case of lost or missing children, ill or injured children, medical emergencies and general emergencies 5) Policy for any administration of medication 6) Procedures for response to natural and man-made disasters

Description: Based on observation and staff interviews, the licensee did not ensure that each restroom area provided for children was equipped with soap, toilet paper, and disposable towels or an air dryer within reach of children. Evidence: During the inspection on October 18, 2016, children were observed during their restroom break and there was not soap available for children to use to wash their hands.

Plan of Correction: The assistant center director will make sure every day the is soap filled.

Description: Based on observation and staff interviews, children under 10 years of age always shall be within actual sight and sound supervision of staff, except that staff need only be able to hear a child who is using the restroom. Evidence: During the inspection conducted on October 18, 2016 , two children, age two were observed running down a hallway away from the childcare staff and around the corner. The hallway was approximately 75 feet long. The children were not within the site supervision of staff for approximately 2 minutes.

Plan of Correction: The assistant center director will work with the director to make sure children are staying in a line.

Description: Based on a documentation review and staff interviews, the licensee did not ensure that for children 16 months old to two years there was one staff member for every five children. Evidence: During the inspection on October 18, 2016, it was confirmed through documentation that on the following days the child to staff ratio was inadequate; 1) On September 23, 2016, from 2:30-until 4:07 there were 11 children and the age range was 23 months to age 2 with only 2 staff. Three of the children were 23 months, child #1, child #2 and child #3. 2) On September 27, 2016, from 8:50-until 12:30 there were 13 children and the age range was 23 months to age 2 with only 2 staff. Four of the children were 23 months, child #1, child #2, child #3 and child #4. 3) On October 4, 2016, from 8:45-until 12:30 there were 11 children and the age range was 23 months to age 2 with only 2 staff. Three of the children were 23 months, child #1, child #2 and child #3.

Plan of Correction: The assistant center director will work with the director to make ensure ratio are kept up to licensing.

Complaint related: No
Description: Based on observation and staff interviews, the licensee did not ensure that staff and children's hands were washed with soap and running water after toileting. Evidence: During the inspection on October 18, 2016, a child and a staff were observed during the diaper changing process and neither the staff or the child washed their hands with soap after diapering.

Plan of Correction: The assistant center director will work with the director to properly train staff and make sure licensing standards are clearly explained to teachers.

Description: Based on observation and staff interviews, the licensee did not ensure the diapering standards were followed. Evidence: During the inspection on October 18, 2016, the diapering process was observed and it was determined the following diapering standards were not followed; 1) Two children were observed being changed and the surface used for diapering or cleaning the children was not cleaned with soap and at least room temperature water and sanitized after each use. 2) There was not a leakproof covered receptacle for soiled linens. 3) There is not a foot operated traschan for disposable diapers that are to be disposed of in a leakproof or plastic-lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

Plan of Correction: The assistant center director will work with the director to properly train staff and make sure licensing standards are clearly explained to teachers. Will purchase a trashcan.
