Comments:
An unannounced monitoring inspection was conducted on 11/2/16 at 9:15 a. m. and concluded at 12:15 p. m. The inspector met with the program director and some but not all Child Day Center standards were reviewed. The following staff-to-child ratios and activities were observed: Infant A (6wk-8m) Seven infants with two staff. The infants were having free play. Infant B (8m-16m) Eleven infants with three staff. The infants were having free play. Toddler (16m-24m) Ten children with two staff. The children were observed having free play outside. Two Years There were 12 children with two staff. The children and staff were observed having circle time and singing songs. Preschool IA (2 1/2 years-3 years) There were 14 children with three staff. The children were having free play, painting and stories on the carpet. Preschool 2A (Young 3 years) There were 12 children with two staff. The children were having centers and later, the children and staff were observed playing with puzzles, blocks, rice table with pouring utensils, computer, home living and art. Preschool 2B (3-4 years) There were 13 children with two staff. The children were sitting down writing their names, later the children were being told a story about The First Thanksgiving. Pre-K (4-5 years) There were 18 children with two staff. The children were observed playing in home living, computer station, blocks, paint and working on words. Lunch consisted of: Meat Balls Pasta with tomato sauce Corn Pineapple Milk Three staff records were reviewed. Two children records reviewed. Three children medications reviewed. Several staff are trained in MAT (medication administration trained). Conversations were held with several children and staff. Two classroom observations conducted. All classrooms are nicely equipped with a variety of equipment, toys and supplies. The most recent evacuation drill was documented on 10/19/16. The most recent shelter-in-place drill was documented on 6/30/16.

Standard #: 22VAC40-185-290-3
Description: Based on observation the center did not have all outlets capped. Evidence: On 11/2/16, the inspector observed a power strip under the computer station in the Preschool 2 B class that had uncapped outlets.

Plan of Correction: This was corrected while the inspector was on site.

Standard #: 22VAC40-185-570-E
Description: Based on observation the center did not have all infant formula labeled as required by standards. Evidence: On 11/2/16, there were infant bottles being stored in the refrigerators in both infant classrooms that were not labeled with the date.

Plan of Correction: This was corrected while the inspector was on site.
