An unannounced monitoring inspection was conducted on Wednesday, October 26, 2016, from approximately 9:50am until 1:40pm. Present today were: Eleven infants with three staff - children were observed during floor play, diapering and while sleeping Nine toddlers with three staff Nine two year old children with two staff - children were playing with legos and carving a pumpkin 2B= nine children with two staff - children were decorating pumpkins they had found in the pumpkin patch 3A= twelve children with two staff - children were creating with clip sticks and connectors, nuts and bolts, and stack on tools 3B= eight children with two staff - children were practicing math skills 4A= ten children with one staff- children were creating leaf creatures, discussing the continents and building with legos 4B= thirteen children with two staff - children were practicing hand writing, playing Jr. Boggle and had created "g" giraffes Lunch: Salisbury steak, mashed potatoes, peaches, crackers and milk Six staff records, five children's records and five children's medications were inspected.
Standard #: 22VAC40-185-70-A Description: Based on review of staff records, the center did not ensure all requirements for employment were met. Evidence: Out of six staff records inspected, Staff #2, with a start date of 9/12/16, does not have the two references required prior to employment. Staff #3, with a start date of 9/22/16, has references dated after employment; one reference dated 10/13/16 and a second referecne dated 10/14/16. Plan of Correction: Per Center: "References obtained on 10/28/16. In the future references will be obtained before employment."
Standard #: 22VAC40-185-330-B Description: Based on inspection of the resilient material required for playground equipment, the center did not ensure the required amount of the material was present. Evidence: The center has added swings on the back pre-school playground. The area in front of the swings has two areas with no resilient material. The large slide on the preschool playground had three inches in depth of resilient material. Based on the height of the slide, six inches would be required. On the school age playground, the area under the swings measured a depth of four inches of mulch, however, it required six inches in depth of the resilient material. Plan of Correction: Per Center: "Mulch delivered and placed on 11/1/16."
Standard #: 22VAC40-185-340-F Description: Based on observations, the center did not ensure children under the age of ten years were always within actual sight and sound supervision. Evidence: The licensing inspector observed the following in the infant classroom: Three staff were present with eleven infant children. One staff was in the closed diapering room changing one child, a second staff was sitting on the floor with a group of children and the third staff was standing by the sink area. Children were sleeping in the area of the classroom when first entering this "L" shaped room. One child was in a crib placed by the right side wall and was not within sight supervision. Standard #: 22VAC40-185-510-A Description: Based on inspection of medications present for children, the center did not ensure each parental authorization to dispense the medication was complete. Evidence: Out of five medications inspected during the inspection, medication #3 parental permission for the center to dispense the medication expired 5/15/16. Medications #4 and #5 permission paperwork was for the public school system, not the center. Plan of Correction: Per Center: "Medication forms are corrected."
