An unannounced monitoring inspection was conducted during the morning program on November 3, 2016. There were 18 children present with two staff members. The children were observed in their classroom as they participated in small group activities, circle time and morning snack. The children also played outdoors on the play ground prior to lunch time. Staff were observed interacting with the children and provided redirection as needed. A sample of 5 records was reviewed, 4 children's and 1 partial staff record. Documentation of the annual fire and health inspections and the practiced emergency drills was reviewed. A prescribed medication was observed, staff have completed medication administration training (MAT). Findings of the inspection were reviewed with the Lead Teacher. Standard #: 22VAC40-185-140-B Description: Based on a sample review of children's records, the Center failed to ensure documentation of a physical examination accepted at enrollment was within the prescribed time period; within 12 months prior to attendance for children two years of age through five years of age. Evidence: One of four reviewed records did not have documentation of a current physical exam. The date of the physical exam for child #3 (3 years-old) was dated 07/01/15, the record noted the child was enrolled on 08/17/16. Plan of Correction: The responsible staff will closely monitor dates to ensure the PE is within the required time frame. The Center will request documentation of an updated annual physical exam.
Standard #: 22VAC40-185-560-K Description: Based on direct observations, staff failed to follow proper sanitizing procedures on the tables used by the children at snack time. Evidence: The LI observed the staff member spray the tables with a bleach/water solution and immediately wipe the table top with a paper towel. The solution was not allowed to air dry for at least 2 minutes before drying with a paper towel. Plan of Correction: Staff will be reminded to follow proper sanitizing procedures.
