- Technical Assistance:
1. Reviewed types of items considered to be hazardous and the required storage.
Conducted an unannounced monitoring inspection 1:10-2:30pm. Observed 60 children + 11 direct-care staff. Ratios and supervision were in compliance. A new Director was hired in August 2016. Children were observed having outdoor free-play, Spanish, napping and having quiet time. Staff were actively engaged with the children, providing active supervision, and tending to the needs of those having difficulty napping. The school was found to be clean, and well supplied with developmentally appropriate toys and equipment for the children. Catered food services are provided as an option for those not wishing to bring food from home. Required documents posted. Medication management and record keeping violations found. Standard #: 22VAC40-185-160-A Description: Based on records reviewed, it was determined that 1 of 3 staff records did not include a TB screening completed within 12 months prior to or 21 days after employment. Evidence: The TB test on-file for Staff #5 was dated March 2015, and the hire date was September 2016. Plan of Correction: The updated TB form was filed.
Standard #: 22VAC40-185-70-A Description: Based on records reviewed, it was determined that 1 of 3 staff records did not include required information. Evidence: There was no documentation of orientation training for Staff #1, and there was no documentation of education as needed for the position she was hired to work in August 2016. Plan of Correction: Orientation form was completed & transcripts/diploma requested
Standard #: 22VAC40-185-270-A Description: Based on observations made, it was determined that an area of the center was not maintained in a safe and operable condition. Evidence: A downspout, located adjacent to the tryke riding track, had peeling and chipping paint, as did some areas of the building it was attached too. Plan of Correction: The downspout was sanded and painted.
Standard #: 22VAC40-185-290-3 Description: Based on observations made, it was determined that electrical outlets did not have protective covers. Evidence: Two electrical outlets in the stairwell did not have covers. Plan of Correction: Outlet covers replaced
Standard #: 22VAC40-185-330-B Description: Based on observations made, it was determined that an unsafe fall zone was present at the bottom of one of the slides. Evidence: A tree root was exposed at the bottom of a slide. Plan of Correction: Mulch has been upgraded to cover exposed root
Standard #: 22VAC40-185-450-A Description: Based on observations made, it was determined that there were no linens for children sleeping on nap mats. Evidence: There were 4 children in the Undercroft classroom observed lying directly on the nap mats with no top or bottom linens. Plan of Correction: Extra linens are available for children who forget theirs.
Standard #: 22VAC40-185-510-A Description: Based on records reviewed, it was determined that there was no written parental authorization for Brooksfield School staff to administer 1 of 3 prescription medications on-site. Evidence: A form giving Fairfax County Public Schools staff permission to administer an emergency medication belonging to Child #1 was signed/dated by the parent on 6/29/16. Plan of Correction: A Brooksfield authorization form was completed & signed.
