Technical Assistance:
Discussed following a calendar year for evacuation drills.

Comments:
An unannounced monitoring inspection was conducted today beginning at 10:20am. There were 56 children, ages 3-5, directly supervised by 9 staff. The physical plant, 3 staff records, 4 children's records, evacuation drills, emergency supplies and policies were inspected. An asbestos report, dated 5/9/2016, was made available for review. Children were observed on the playground, participating in group activities, lining up, interacting with their caregivers and eating snack. The children bring their own food from home and bags were labeled and dated properly. There was an abundant supply of developmentally appropriate activities, artwork was displayed and the children engaged in a conversation about their turtle named Yurtle. Standard #: 22VAC40-185-160-C
Description: Based on center management records, a TB screening for 1 out of 3 staff records reviewed had expired. Evidence: TB screening for Staff #1 expired on 9/15/16.

Plan of Correction: TB report was unavailable on date of visit. Nurse have placed in necessary file, the current report (attached).

Standard #: 22VAC40-185-330-E
Description: Based on review, it was determined the facility does not cover the sandbox located on the playground when not in use. Evidence: No cover was located outside and staff members stated the previous covers were broken and unusable.

Plan of Correction: New cover has been ordered and will be installed with replacement available as well.
