Discussed with center management: The fence on the older playground and multiple climber toys on the younger playground that are in need of repair. Work orders have already been placed for all repairs at the time of inspection.
An unannounced renewal inspection was conducted today. Observed 67 children directly supervised by 13 staff members. Ratios and supervision were in compliance. A sample of 4 children's records and 4 staff records were also reviewed. The physical plant, medications, emergencies site plans and procedures were also reviewed. The children were engaged in various activities, to include: free play, reading, art projects, infant care, feeding and diapering. The two's were all waiting for the garbage truck to leave before heading outside on the playground. Areas of non compliance are identified in this report. The exit interview was conducted with the Director. Inspection conducted 9:30 am-11:20am. Standard #: 22VAC40-185-500-A Description: Based upon observation, proper hand washing procedures were not followed after feeding a baby. Evidence: After feeding a baby their bottle on the floor, staff A did not wash their hands before rejoining the group. Plan of Correction: Assistant Director reminded staff of hand washing procedures at the time of inspection.
Standard #: 22VAC40-185-540-D Description: Based upon observation and interview, required emergency supplies were not available during the inspection. Evidence: Staff were unable to locate the center's first aid kit and weather radio during the inspection. Plan of Correction: The site is going through a re-fresh and the office where the emergency supplies were usually kept had been packed up and items were stored in a room next door. The process had taken longer than anticipated. Supervisors will locate required first aid supplies and make them accessible.
Standard #: 22VAC40-185-550-L Description: Based upon records reviewed, required information was found to be missing or incomplete. Evidence: 6 of 6 incident reports were missing a description of "future action to prevent reoccurrence" of injury as required and the time and date parent were notified of incident. Plan of Correction: Site safety coordinator will retrain staff on proper documentation. The form being used at the center is outdated.
