Comments:
An unannounced monitoring inspection was conducted today from 8:30 - 9:30 A.M. There were 50 children present with 4 staff. The children were getting ready to go on field trips to two locations. The children were observed taking turns using the Wii game system. The exit interview was conducted with the Center Supervisor. The violation Notice will be e mailed for completion.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, the facility staff did not obtain updates on the 2015 - 2016 school year Plan of Action for children with allergies/medical conditions. Evidence - The record for one child with serious medical conditions contained a Plan of Action that had not been updated since 10/6/2015.

Standard #: 22VAC40-185-340-A
Description: Based on observation, the facility staff did not ensure the protection of children going on a field trip. Evidence - There was at least one child on the school bus who had an epi pen on site for serious allergies. The staff closed the SACC rooms and left to get on the bus without the children's emergency medications. When the licensing inspector asked where the medications were, the teacher went back into the SACC room and got the medications.
