Technical assistance was provided in the following areas of the standards: 22VAC40-185-(6)-440 (Crib Spacing)

Comments:
An unannounced monitoring inspection was conducted on 11/18/16 from 9:15am - 12:20pm. During the inspection there were 29 children ages five months to five years old in care with seven staff. A tour of the facility was conducted and children were observed in a variety of activities in the classrooms, playing outside, and eating lunch. Records were reviewed for five children and two staff. Medication, emergency procedures and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, it was determined that the facility did ensure that all areas and equipment of the center, inside and outside, are maintained in a safe and operable condition. Evidence: 1. On the playground, there were five sections of the wooden exterior fence that were broken. 2. Staff #3 (Program Director) confirmed the fence was broken.

Plan of Correction: The facility responded: A maintenance request is in place for repairs.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of two staff records and interview, it was determined that the facility did not deny continued employment of a staff who did not have a search of the central registry finding within 30 days of employment. Evidence: 1. The record for staff #2 (date of hire 8/24/16) did not have documentation of a completed search of the central registry finding. Staff #2 was present during inspection and working with children. 2. Staff #3 (Program Director) confirmed that she had not received a completed search of the central registry finding for staff #2.

Plan of Correction: The facility responded: We will call on Monday to check to see if they have received it. We will make sure they send it back in a timely manner. If not we will call if it is not back within a 2 week window.
