The importance of a thorough injury report was discussed with the program director.

The licensing inspector conducted an unannounced complaint inspection on 11/15/16 in response to a complaint that was received in the licensing office on 11/09/16 regarding allegations in the area of special services and policies. Interviews were conducted with staff , records were reviewed and observations of the program were made relating to the allegation. The information gathered during the investigation does not support the allegations, so the complaint is determined to be NOT VALID. A monitoring inspection was also conducted and other violations were cited, not related to the allegations.

Description: Based on a record review, the center failed to maintain all required information when recording children's injuries. Evidence: 1. On 11/09/16, child #1 had a written injury report that did not have a staff and parent signatures or two staff signatures. The document had only 1 staff signature. 2. On the same injury report there was no documentation how the parent was notified or the time.

Plan of Correction: On 11/17/2016 all staff members were retrained on incident reporting and how to handle if a parent refuses to sign the incident report. We also will ensure that we complete the entire report even when attaching a behavior plan or other items to incident reporting. Managers will ensure they sign the report only when all pieces of report are complete. District Manager will periodically inspect to ensure compliance.
