Comments:
10/31/16 An unannounced inspection was conducted jointly with another agency in response to a self-reported incident called into the licensing office concerning supervision. Based on interviews and a review of documents, the evidence supports the allegations. A violation notice will be issued. Thank you for your cooperation. Please contact me if I may be of service.

Violations:
Standard #: 22VAC40-185-340-F
Description: Based on Staff interviews and a review of documents, the facility failed to provide sight and sound supervision of all children in care. Evidence: 1. On 10/27/16 a group of preschoolers were playing on the playground. 2. Staff #2 and 3 reported that they took the children inside, counting them as they entered the room, that child #1 went back outside into the fenced play area and then the door was closed. 3. A teacher from another classroom (Staff #1) saw the child out the window, retrieved the child and then returned the child to the appropriate classroom. 4. The time frame the child was outside alone is reported to be 30-45 seconds.

Plan of Correction: Staff # 2 & #3 were given an Improvement Plan to prevent reoccurrence. Additional safety measures were put into place for all classrooms to have administration double check all transitions in and out of classrooms or movement to new locations in building as an added layer of safety. Teachers have been trained to be in front and back of transitions lines to keep children from slipping out of line.
