Comments:
15 Children were in care during the inspection, The age range of the children was two years to four year old. The children were observed during rest time. Eight children and six staff records were reviewed. Fire inspection 2/9/16 health inspection 2/9/16. Inspection time 11:45am-1:30pm

Standard #: 22VAC40-185-270-A
Description: Based on observation of the playground, The center did not have mulch covering the weed block. Evidence: The weed block is sticking up from the mulch under the piece of equipment called the vortex posing a tripping hazard.

Plan of Correction: The weed block will be removed from the play area and mulch will be added. The equipment will be off limits until it is fixed.
