An unannounced monitoring visit was conducted on December 5, 2016 from approximately 12:50 to 3:21pm, to measure some, but not all, of the licensing standards. The following ratios were observed:
Infants: 2:6
Toddlers: 2:9
Two's: 2:13
Preschool 1: 2:15
Fours/Fives: 2:15
Upon arrival, the children were observed napping. At that time, the infants were observed playing on the floor and napping. Later, the children were observed waking from nap, washing hands, and eating a snack of vanilla wafers and milk. The infants that were awake, were eating Cheerios and vanilla wafers and milk. Five children's records and emergency information and training for 7 staff persons was reviewed. Staff records are maintained at a central location. All areas of the facility including the kitchen, classrooms, hallways and playgrounds were inspected. The first aid kit and emergency supplies were also reviewed. The medications and accompanying authorizations were reviewed. The most recent fire drill was 11/30/16. The most recent shelter in place drill was conducted 2/24/16.

Description: Based on a review of a sample of children's records, the facility did not ensure that documentation of immunizations was obtained every 6 months for children under the age of two. EVIDENCE: The record of child # 5, a 16 month old with a start date of 10/12/15, contained an immunization record dated 10/6/15. No additional documentation of immunizations was on file.

Plan of Correction: Corrected on site.

Description: Based on a review of documentation at the facility, the facility did not ensure that evidence of insurance coverage was available for review. EVIDENCE: The facility did not have evidence of current insurance coverage on site at the facility. The declaration on file expired 9/1/16.

Plan of Correction: A copy of the insurance will be maintained on site.
