Technical assistance was provided in the following areas of the standards: 22VAC40-185-(4)-270 (Building maintenance); 22VAC40-185-(4)-330 (Play areas)
An unannounced monitoring inspection was conducted on 12/7/16 from 9:00am - 10:45am. At the time of entrance there were 49 children ages eight months to five years old in care with 11 staff. Children were observed participating in various classroom activities and playing outside. Records were reviewed for five children and three staff. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.
Standard #: 22VAC40-185-500-B Description: Based on an observation of the diapering process, it was determined that the facility did not ensure that disposable diapers are disposed in storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Evidence: 1. The Licensing Inspector found three blue plastic trash bags that each contained a soiled diaper on the ground outside door that goes from the older infant classroom, also know as Red #1, to the playground. 2. Staff #2 (Assistant Director) confirmed that the three diapers were not disposed of properly. Plan of Correction: All teachers will be re-trained on how to properly disposed of diapers by using a hands free method.
Standard #: 22VAC40-191-60-C-2 Description: Based on a review of tthree staff records, it was determined that the facility did not deny continued employment of a staff who did not have a search of the central registry finding within 30 days of employment. Evidence: 1. The record for staff #4 (date of hire 10/26/16) did not have documentation of a completed search of the central registry finding. Staff #4 was present during inspection and working with children. 2. The record for staff #5 (date of hire 10/26/16) did not have documentation of a completed search of the central registry finding. Staff #4 was present during inspection and working with children. 3. Staff #1 (Program Director) confirmed that she had not received a completed search of the central registry finding for staff #4 or staff #5. Plan of Correction: We will follow up by contacting the Department of Social Services to ensure that both background checks were received and obtain their status. For all new employees, we will ensure that we contact the appropriate office if the background check is not received within 30 days.
