22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was conducted from 9:30 a.m.-12:15 p.m. with the center assistant director. 51 children, ages 8 months through 5 years old, were under the care and supervision of nine staff. Children were observed having circle time, napping, being held and fed a bottle, playing with toy cars and blocks, building with Magnatiles, and playing a "My Community" memory game. Both the fire and health inspections are current. The last emergency escape drill was 11/11/16. The last shelter-in-place drill was 9/14/16. Three medications and two staff records were reviewed. Standard #: 22VAC40-185-340-A
Description: Based on review of of three medications, the staff did not ensure the care and protection of a child in care. Evidence: Child A's, age 8, emergency medication, Q-dryl liquid antihistamine, for food allergies expired at the end of October 2016.

Plan of Correction: The assistant director will notify the parent today to replace the expired medication. The medication had not been opened/used, and our center does not serve those foods that Child A is allergic to.

Description: Based on review of written attendance record, review of four children's records, staff interviews, and observation, the center did not maintain the required supervision ratio of staff to children. Evidence: Two staff members were supervising 12 children in the two's classroom. The required ratio of one staff member for every five children , based on the youngest child in the group, Child B (date of birth: 12/16/14; age 23 months) applies to the entire group.

Plan of Correction: The assistant director immediately returned Child B to the toddler classroom. In the future, children will not be transitioned without the required three components of the written parent permission, the assessment of the director, and the assessment of the lead teacher indicating that all three individuals feel that the child is ready for the transition.

Description: Based on review of three staff records, not all central registry record checks were updated every three years. Evidence: Staff A?s last completed central registry record check was dated 10/21/13.

Plan of Correction: Staff A's central registry record check request had been sent in but returned with a request to clarify a deceased spouse's information.
