- Technical Assistance:
1. Reviewed requirement for all documents used to qualify staff to be translated into English. 2. Discussed importance of accuracy on daily reports. 3. Discussed teacher interactions and prompting of children in one classroom. 4. Sink drainage in the kitchen should be monitored to ensure a safety hazard does not develop. 5. Reviewed requirement for milk to be served at lunch vs. offered to children.
Conducted an unannounced renewal inspection. Observed 25children + 7 direct-care staff. Ratios and supervision were in compliance. Children were observed washing hands, eating lunch, being diapered, taking naps, reading books, and having indoor free-play during the inspection. Staff were actively engaged with the children and promptly tending to their needs. Lunch served: chicken nuggets, sweet potato tots, broccoli, ranch dip, ketchup, and milk/water. The center was found to be very clean, safeguarded appropriately, and exceptionally well supplied with toys and equipment for the children. No medications reported to be on-site. Standard #: 22VAC40-185-160-A Description: Based on records reviewed, it was determined that 1 of 4 staff records did not include a TB screening completed within 12 months prior to the date of hire, or 21 days after. Evidence: Staff #9 was hired 8/19/16, and the TB test on-file was completed in June 2015. Plan of Correction: TB test has been updated and is now on file.
Standard #: 22VAC40-185-70-A Description: Based on records reviewed, it was determined that 3 of 4 staff records reviewed did not include required documentation. Evidence: 1. Staff #7 - There were no dates documented on previous work experience that would allow for calculation to determine the minimum requirements were met. 2. Staff #8 - The was no documentation of previous work experience, as that section of the job application was left blank, and determination of the minimum relative work experience was not determined. 3. Staff #9 - There were no dates documented on previous work experience that would allow for calculation to determine the minimum requirements were met. Plan of Correction: Work experience has been provided by staff to ensure minimum requirements were met and are now on file.
