Technical Assistance:
1. Discussed injury report requirements and availability of emergency posting and staff DHO records.

Comments:
An unannounced monitoring inspection was conducted today from 3:25pm-4:30pm. There were 35 children directly supervised by 3 staff. The physical plant, 1 on site staff record, 5 children?s records, 2 medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed having snack and doing homework. Snack preparation was also observed. There was a sufficient supply of materials for the children. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-550-E
Description: Based on record review, the center did not maintain record or dates of the practiced drills. Evidence: 1. The last recorded emergency evcauation drill was listed as 10/1916. There was no November evacuation drill recoreded.

Plan of Correction: Staff reports an evacuation drill was conducted in November but had not been recorded. The November date was recorded during the inspection.
