Comments:
An unannounced non-mandated monitoring inspection was completed on 12/8/16 as a part of the conditional license period. There were 13 children present with two staff supervising. Four children's records and two staff records were reviewed. The inspector arrived at the center at 8:45 am and departed at 12:30 pm. The children were observed during breakfast, during free time, during a special craft activity (gingerbread houses) with parents, during outside time, and during preparation for lunch. There was discussion with the teacher about the following: the work phone number for the father of child #4, the outside light on the front entrance at steps, the occupancy listed on the building evaluation form only allows for the number of children and two teachers (parent activity added nine extra adults and three extra children) - will contact building official for question of revising the form and the center should follow up to ensure compliance with the building code , concerns of fire code issues to be discussed with the local fire official, the current Sworn Statement for staff #1.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that all hazardous substances were kept in a locked place. Evidence: 1. There was a storage room directly by the classroom that was unlocked and contained the following hazardous substances: two cans of aerosol spray insecticide, two large tubes of caulking, two large tubes of drywall glue that had been opened with dry substance exposed, three propane tanks (heavy and appeared to be full), a gallon jug of brand glass cleaner, a gallon jug of antimicrobial hand soap, a gallon jug of all purpose cleaner, a spray bottle of degreaser, an aerosol spray air freshener. The two doors to enter the storage room had non-locking door knobs.

Plan of Correction: All items were removed from the premises during the inspection. The teacher will check the storage area regularly to ensure that there are not any hazardous substance stored in the room.

Standard #: 22VAC40-185-320-B
Description: Based on observation and measurement, the center failed to ensure that the hot water in the rest room areas was maintained at a temperature below 120 degrees Fahrenheit. Evidence: 1. The hot water was observed to be very hot. The inspector used a digital thermometer to measure the temperature of the hot water. There are two rest rooms (boys and girls) used by the children. The girl's rest room hot water measured 140 degrees and the boy's rest room hot water measured 139 degrees.

Plan of Correction: The teacher will contact administration who will contact the building owner to have the temperature on the hot water turned down to below 120 degrees.

Description: Based on observation and interview, the center failed to ensure that the findings from the most recent inspection were posted. Evidence: 1. The "Violation Notice" was not posted at the center. The teacher was asked about the documents including the "Violation Notice" and the teacher stated that she did not have any documents to post from the last inspection.

Plan of Correction: The teacher will post the "Violation Notice" from this inspection and ensure that it remains posted until the next inspection.
