Technical Assistance:
There is an area for potential tripping near the outdoor seating area where the exposed stream bed has several roots. In the two year old class, four large strands of loose yarn approximately 3 feet each were observed on the floor in different areas during the inspection. While they were from stitching toys, they were not located near those toys but spread throughout the room on the floor throughout the inspection.

An unannounced monitoring inspection was conducted today from 10:30 am - 1 pm. There were 42 children present in 6 classrooms with 7 direct staff. I observed children involved in circle time; singing a song; engaged in dramatic play; listening to a story and eating lunch. Responses to violations were dictated from the assistant director today. Standard #: 22VAC40-185-270-A
Description: During today's inspection, some areas and equipment were found not to be maintained in a clean, safe and operable condition. Evidence: Indoors: There was noticeable peeling paint observed in areas of the classrooms, especially on the door frame of the toddler room. There was also a wall protector (for doorknob damage) observed on the wall in the toddler room that had crumbling soft rubber coming off it, which could present a choking hazard. In the same room, a toy shelf had a splintering area on the lower part of the structure. Outdoors: There was peeling surface observed at the edge of the preschool slide. There were also protruding nails observed in wood fencing accessible to children and a broken plastic frame on the bottom of a preschool "kitchen" structure outdoors. An area of the metal fencing is "bowed out" at the bottom, posing a risk.

Plan of Correction: Work orders have been in for painting. In the meantime, areas will be taped so children do not peel it the paint. A work order was put in for the outside fence and will be put in for the wall protector. I will hammer the nails and tape the pieces of the toy shelf.

Description: During today's inspection, hazardous substances were observed to not be kept in a locked place as required. Evidence: The door to the laundry room where cleaning chemicals were stored was observed not to be locked while children were present in the classroom today..

Plan of Correction: Corrected during inspection. All staff will be reminded to lock the door.

Standard #: 22VAC40-185-420-E-1
Description: During today's inspection, it was observed that a daily record on a infant (who cannot turn over by himself) did not include the amount of time spent on his stomach as required. Evidence: A review of the daily records for Child #5 found no documentation of "Tummy time" on any recent days of attendance.

Plan of Correction: Teachers will be reminded.

Standard #: 22VAC40-185-430-B
Description: During today's inspection, some children were observed playing with toys that were not age and stage appropriate for them. Evidence: In the two year old classroom, several children were observed playing with miniature metal cars with tiny wheels (approximately 1/4 inch.) One child's car had no tire, and a sharp metal stem where the tire had been attached was observed to be protruding. Six of these type of cars were observed in the classrooms.

Plan of Correction: These were for the transportation unit but they will be removed. Teachers will be retrained.

Standard #: 22VAC40-185-510-A
Description: A review of children's medications today found some standards were not met as required. Evidence: The authorization for Child # 1 and #2 for prescription allergy medication did not grant permission to the school to administer the drugs. Additionally, the over the counter medication for Child # 3 did not have a dose written on it.

Plan of Correction: The parent has been contacted about the two medications, and the dosage was written on the bottle of Motrin today.

Standard #: 22VAC40-185-550-M
Description: A review of incident/injury forms today found that not all entries contained all required information. Evidence: Of a sample of five reports reviewed, four on 10/17, 11/7, 11/11, 11/23 did not contain the time and method the parent was notified.

Plan of Correction: Teachers will be retrained.
