22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was conducted today at 11:25am. There were 11 preschoolers (4s) directly supervised by 3 staff. The physical plant, 3 staff records, 4 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in lunch and preparing for nap time.. There was an abundant supply of books, toys, and materials for the children. The center was clean and organized. Areas of non-compliance are identified in this report. Please sign and date all documents and return it to me within 10 calendar days from today. Description: Based on review, the facility failed to ensure that the injury prevention procedures are updated at least annually based on documentation of injuries and a review of the activities and services. Evidence: Reviewed the injury prevention procedures and found that the last review was dated January 2015.

Plan of Correction: It will be corrected by Thursday.

Description: Based on review, the facility failed to maintain documentation of an annual fire inspection report. Evidence: Reviewed the fire inspection documentation on site and found that it was dated to expire 3/20/15.

Plan of Correction: It will be corrected on Friday.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: Observed peeling paint along the "white" wall in the rear of the classroom.

Plan of Correction: I will call maintenance.

Description: Based on observation, the facility failed to ensure that hazardous substances are kept in a locked place using a safe locking method that prevents access by children. Evidence: Observed 3 hand sanitizer bottles accessible to children (2 by the entrance and 1 behind the "play house" set up), all marked keep out of reach of children.

Description: Based on observation, the facility failed to ensure that electrical outlets have protective covers. Evidence: Observed 3 uncovered electrical outlets accessible to children on a power-strip underneath the second computer desk.

Description: Based on observation, the facility failed to maintain the minimum items in the first aid kit. Evidence: Observed that there were no ice packs in the first aid kits or freezer.
