Comments:
The licensing inspector conducted an unannounced monitoring inspection Wednesday, December 28, 2016 from 10:15am to 11:50am. This is a monitoring inspection, as well as to follow up on non compliance from the previous complaint inspection on 10/25/2016. The children were observed engaged in a variety of activities. The infants and toddlers were observed listening to music and playing with toys. The Preschoolers were observed tracing the letter Jj, while the Twos were working on a puzzle and the School Agers were making a snowman craft. A formal observation was completed with the Preschool group. The lunch menu was posted, and lunch is to consist of salisbury steak, mashed potatoes, peas, applesauce, buttered toast and milk. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today as well as the facility?s van. Three children?s records and one employee record were reviewed during this inspection. Medication is not being administered. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 11/30/2016 Shelter-in-place drills: 7/29/2016 & 4/29/2016 Last fire inspection: 07/14/2016 Last health inspection: 04/20/2016 Total enrollment today: 24 Today, the following child to staff ratios were observed: Infants & Toddlers ?4:1 Twos ? 5:1 Threes & Fours ? 9:1 School Age ? 6:1 The violations from the previous inspections were checked for corrections. Description: Based on a review of children's records on 12/28/2016, the center did not ensure that two of three records reviewed contained a work phone number for the place of employment listed for each parent. EVIDENCE: 1. The record of Child #1 did not contain a phone number for the place of employment listed for the child's mother and father. 2. The record of Child #3 did not contain a phone number for the place of employment listed for the child's mother and father.

Plan of Correction: Will have parents complete today. Going forward will have parents provide a work number or put n/a in that space.
