Comments:
An unannounced monitoring inspection was conducted at the facility from 10:05am-11:30am. A tour of the facility, a record review and interviews were conducted. A sample of 3 staff and 4 children's records were reviewed. Children were observed during music and movement, playing table toys and blowing bubbles with staff. Infants were observed during diapering and playtime on the floor.

Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation the center failed to ensure hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. In the toddler room there was a spray bottle of bleach and a spray bottle of disinfectant spray in an unlocked cabinet at the bottom of the changing table. 2. In the 3 year-old room there was an opened box of Borax laundry powder in an unlocked cabinet in the bathroom area and 3 spray bottles of disinfectant in an unlocked cabinet above the hand washing sink directly outside of the bathroom.

Plan of Correction: All teachers will ensure that all cabinets and areas are locked and secure at all times. The safety captain will do daily inspections in the classroom to ensure compliance. Management will do daily walkthroughs to ensure compliance. DM will inspect compliance during school visits. Borax was moved to a locked cabinet during time of visit.

Standard #: 22VAC40-191-60-C-2
Description: Based on a record review the center failed to ensure each employee had the findings of a central registry check within 30 days of employment. Evidence: Three of the 3 staff files reviewed did not have documentation of having a central registry check within 3 days of employment. Staff #1 (hired 11/08/16), Staff #2 (hired 11/21/16) and Staff #3 (hired 11/21/16)

Plan of Correction: During New Employee Orientation all background checks and state required checks will be completed and sent out on Day one of employee hire. Employees will not be able to continue employment without required copies on file. Management will ensure periodic tracking to make sure all required documents are updated as necessary. District Manager will inspect all new employee files on every school visit.
