Comments:
An unannounced inspection was made on 1/5/2017 at approximately 10:00 a. m. in response to self-reported a concern received by our department on 12/28/2016. The concern the center reported was inadequate supervision of two children. Interviews were conducted with appropriate staff and classroom furniture arrangements were checked and discussed. Based on the information provided there was a violation cited regarding supervision, so the concern reported was determined valid.

Standard #: 22VAC40-185-340-F
Description: Based on interviews with staff the center did not provide sight supervision of children at all times. Evidence: On 12/28/2016, at approximately 11:00 a. m., two children (age four years) were not in direct sight supervision of staff. The classroom room arrangement created a blind spot in the Library/Book Center, and the supervising staff were unable to see interactions between two children.

Plan of Correction: Effective 12/28/16 furniture in the classroom was rearranged to assure there are no blind spots. This allows all children to be visibly observable at all times.
