22VAC40-191 Background Checks (22VAC40-191)
An unannounced renewal visit was done today January 5, 2017 beginning at 3 PM and ending at 4:45 PM. There were 21 children and 2 staff present today. The children were observed during snack, Christmas discussion and homework time. They also played outside on the blacktop and play equipment to the rear of the school. The medications were reviewed today as well as a sample of children's files. The staff files were reviewed earlier today at the main office. The violations were discussed today with the child care director as well as the site coordinator. Corrective action was obtained during the discussion.
Standard #: 22VAC40-185-130-A Description: Based on review of five children's records, the center failed to ensure that all records had immunizations record prior to starting the program. Evidence 1. The inspector found that children # 1 and 2 did not have immunization records. Children 1 and 2 started on 1/5/2017. Plan of Correction: The site director will obtain a copy ASAP from the parents.
Standard #: 22VAC40-185-270-A Description: Based on observation the center failed to ensure that all areas and equipment, inside and outside, shall be maintain in a clean, safe and operable condition. Evidence: 1. The inspector observed two nails protruding at the end of the ramp that the children use to enter the building. Plan of Correction: The staff will have the maintenance personal to remove the nail ASAP.
Standard #: 22VAC40-185-280-B Description: Based on observed the, center failed to ensure that all chemicals were locked. Evidence: 1. The inspector observed that the chemicals were unlocked in a bottom cabinet accessible to the children. Plan of Correction: The staff stated that the maintenance staff is in the process of placing a lock on the cabinet.
Standard #: 22VAC40-185-350-C Description: Based on observation, the center failed to ensure that the staff to children ratio for the mixed age group was applicable to the youngest child in the group. Evidence: 1. The inspector observed 21 children in care with two staff members. The youngest child in the grouping was 4 years of age. The children arrived into the program at 3:15 and at 4:00 one children departed and placed the program into ratio. Plan of Correction: The child care director will work on having another staff person to ensure ratio is met.
Standard #: 22VAC40-191-40-D-1-C Description: Based on review of board officer's and agent's records, the center did not obtain the required repeat background checks on all agents . Evidence: 1. During the inspection the inspector reviewed three agent's records and found that agent # 1 most recent Sworn Disclosure was on 8/21/2013, CRC was on 9/11/2013 and CPS was on 9/17/2013. All background checks are required to be repeated every three years. Plan of Correction: The administration staff will submit background check and will monitor dates.
Standard #: 22VAC40-191-60-C-1 Description: Based on review of two staff's records, the center failed to ensure that all CRC background checks were returned within 30 days of hire. Evidence: 1. The inspector observed staff # 1 that was hired on 8/28/2016 did not have a returned CRC background check. Plan of Correction: The site director will contact the state police to check the status and will submit a new one if need be.
