- Technical Assistance:
Training of additional staff in daily health observation/screening training was discussed with the program director. Documentation required in children's records prior to attending was reviewed with the program director.
An unannounced, mandated renewal inspection was conducted today. The inspector arrived at 10:30 AM and departed at 1:30 PM. Morning activities, lunch service and nap time were observed. Injury records were reviewed. It was reported that no medication is administered to children in care and there is no emergency medication stored at the facility for potential administration. A sample size of four children's records were reviewed. Five staff records for current employees and three staff records for previous employees were reviewed. The outdoor playground was not inspected due to inclement weather (recent snowfall). An exit meeting was conducted with the program director during which cited violations were reviewed. Plans of correction were obtained. Please contact the inspector if there are any questions.
Standard #: 22VAC40-185-160-A Description: Based upon review of five staff records and staff interview, the facility has not ensured that documentation of a tuberculosis screening is submitted no later than 21 days of employment and shall have been completed within 12 months prior to or 21 days after employment. Evidence: 1. The record provided for staff 1, employed on 11/28/2016, contains documentation of a tuberculosis screening that was obtained on 6/4/2015, which is not within 12 months prior to employment. 2. The record provided for staff 4, employed on 9/15/2016, contains documentation of a tuberculosis screening that was obtained on 6/26/2015, which is not within 12 months prior to employment. 3. Staff 1 and staff 9 were unable to provide documentation of tuberculosis screenings obtained within 12 months prior to employ for staff 1 and staff 4. Plan of Correction: The facility responded with the following: In the future, administrators will ensure that tuberculosis screenings for newly hire employees are obtained within 12 months prior to employ or within 21 days after employ.
Standard #: 22VAC40-185-60-A Description: Based upon review of four children's and staff interview, the facility has not ensured that the record for each enrolled child contains all required information. Evidence: 1. The record provided for child 1 does not include the street addresses for the two persons designated to be contacted in an emergency if a parent cannot be contacted. 2, Staff 1 and staff 9 were unable to provide documentation of the addresses for the two designated emergency contact persons. Plan of Correction: The facility responded with the following: Facility administrators misunderstood the Standards, thinking that there was a 30 day time frame to obtain the required information. In the future, all required information will be obtained prior to the child's first day attending the facility.
Standard #: 22VAC40-191-60-C-2 Description: Based upon review of five staff records and staff interview, the facility has not ensured that a child protective services (CPS) central registry finding is obtained within 30 days of employment of the continued employ of the staff person is denied. Evidence: 1. The record provided for staff 2 indicates a date of employ of 10/21/2016. Results of a CPS central registry finding were not in the record. There was no documentation of facility follow-up regarding the requested registry finding. 2. Staff 1 and staff 9 were unable to provide the results of a CPS registry finding for staff 2. 3. Staff 2 was on duty with the children during the inspection. Plan of Correction: The facility responded with the following: The CPS registry finding was requested within 7 days of employ. The registry officials were contacted during the inspection are researching the outcome of the requested search. In the future, the CPS registry officials will be contacted and this will be documented prior to the end of 30 days of employ if the results of the finding are not received.
