Comments:
An unannounced renewal inspection was conducted today from approximately 9:45am-11:30am. There were 23 children ( 2 1/2-6yrs.) directly supervised by 3 staff. The physical plant,2 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books and eating snack. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Description: Based on inspection of children's records all required information was not maintained Evidence: Child 4 was missing: Address for 1 of 2 emergency contacts.

Plan of Correction: We will get the information from parents at pick up

Standard #: 22VAC40-185-510-C
Description: Based on inspection of medications on sight all required authorizations were not maintained. Evidence: There was no parent signature for 1 of 4 authorizations for child 5

Plan of Correction: We will get the signature from mom at pick up.
