- Technical Assistance:
The licensing inspector conducted an unannounced monitoring inspection Wednesday, December 28, 2016 from 12:58pm to 02:50pm. This is a monitoring inspection, as well as to follow up on non compliance from the previous complaint inspection on 07/11/2016. It was the center's rest time and the children were observed resting on cots. The infants were observed playing on the floor with their teacher and the School Age children were observed playing outside on the playground. Snack was to consist of sliced apples with soynut butter. The lunch/snack menu was posted. All classrooms and playgrounds were inspected today as well as the facility?s mini buses. Three children?s records and two employee records were reviewed during this inspection. Medication is being administered and was reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill:12/09/2016 Shelter-in-place drills:02/24/2016 & 10/26/2016 Last fire inspection: 01/26/2016 Last health inspection: 10/14/2016 Today, the following child to staff ratios were observed: Infants ?3:1 Toddlers ?2:1 Twos ? 5:1 Preschool (Threes & Fours) ?7:1 School Age ? 8:1 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-160-A Description: Based on a review of staff records on 12/128/2016, the center did not ensure that each staff member submit documentation of a negative tuberculosis (tb) screening. Documentation of the screening shall be submitted no later than 21 days after employment. EVIDENCE: The record of Staff #1, whose date of hire is 09/19/2016, did not contain documentation of a negative tb. The record did contain documentation that a tb test was completed on November 08.2016, however the results of the test were not in the record. Plan of Correction: TB Test was faxed from Dr. and is now in staff file. Procedure put in place to ensure TB tests are completed timely.
Standard #: 22VAC40-185-420-E-3 Description: Based on a review of children's records on 12/28/2016, the center did not ensure that at least annually parent confirmation that the required information in the child's record is up to date. EVIDENCE: The most recent update in the record of Child # 2 is dated 11/20/2015. Plan of Correction: Parent signed file to confirm it is up to date same day as visit. Plan in place to ensure files updated timely.
Standard #: 22VAC40-191-60-B Description: Based on a review of staff records on 12/28/2016, the center did not ensure that an employee must not be employed or provide volunteer service until the agency has the person's completed sworn statement or affirmation. EVIDENCE: The record of Staff # 1, whose date of hire was 09/19/2016, did not contain documentation of a sworn statement or affirmation. Plan of Correction: Corrected on site. Employee completed sworn statement.
Standard #: 22VAC40-191-60-C-2 Description: Based on a review of staff records on 12/28/2016, the center did not ensure that a central registry finding was obtained within 30 days of employment for one of the two records reviewed. EVIDENCE: The record of Staff #1 did not contain documentation of a central registry finding. Staff #1's date of hire was 09/19/2016. Plan of Correction: Central registry was sent 9/21/2016, but returned 10/24. Fixed issue and sent back to central registry.
