Comments:
An unannounced renewal inspection was conducted today from 10:00am-12:30pm. There were 36 children (infants-5yrs.) directly supervised by 8 staff. The physical plant, 4 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, imaginative play, reading books in a "cozy corner", and cutting out snowflakes with teacher supervision. Hand-washing procedures were also observed. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Description: Based on record review, the center did not maintain documentation of viewing proof of the child's identity and age. Evidence: In 4 of 5 children's records, the center did not complete the section documenting viewing proof of the child's identity and age. The center director stated the items were viewed but not documented and copies were made.

Plan of Correction: Going forward the office will view and document proof of child's identity and age.
