22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 2:45 p.m.-5:30 p.m. with the center director. 73 children, ranging in age from one month through 5 years old, were under the care and supervision of 11 staff. Children were observed eating afternoon snack, playing outside, playing with puzzles and books, and reading. Afternoon snack today was wheat crackers and cheese. Required information was posted. Both the fire and health inspections are current. The last emergency escape drill was 12/9/16. The last shelter-in-place drill was 6/29/16. The playground was toured and inspected. Three medications were reviewed. Three staff records were reviewed. Four children's records were reviewed and complete. Standard #: 22VAC40-185-70-A Description: Based on review of three staff records, the center did not maintain complete staff records. Evidence: Staff A's record did not include documentation of two reference checks. Plan of Correction: The director stated that Staff A's references will be checked tomorrow.
Standard #: 22VAC40-185-80-A Description: Based on a review of the written attendance records, the center did not ensure all children were included on the written attendance record. Evidence: Infant A's written attendance record listed six children with eight children physically present; Infant B's written attendance record listed six children with four children physically present; Toddlers' written attendance record listed 11 children with ten children physically present. Plan of Correction: The director corrected the written attendance records during the inspection and will discuss this topic with the Educational Specialist.
Standard #: 22VAC40-185-240-A Description: Based on review of three staff records, the center did not have documentation that staff received orientation training that includes job responsibilities, policies/procedures, playground safety procedures, child abuse and neglect, and confidentiality by the end of their first day of assuming job responsibilities. Evidence: Staff B's (hired 1/9/17) record did not include documentation of orientation training. Plan of Correction: The director stated that Staff B will complete orientation training tomorrow.
Standard #: 22VAC40-185-510-G Description: Based on review of three medications, the center did not have written authorization to administer medication. Evidence: The center did not have a long-term medication authorization form signed by a parent for Child A's medication. Child A's medication did have an authorization form signed by the doctor. Plan of Correction: The parent signed the medication authorization form for Child A during the inspection.
Standard #: 22VAC40-185-550-D Description: Based on review of the emergency response drills record, the center did not conduct monthly emergency escape drills. Evidence: There was no emergency escape drill documented for November 2016. Plan of Correction: The director stated that going forward that monthly fire drills will be conducted and during all periods of a day.
