Comments:
An unannounced monitoring inspection was conducted today, 1/26/17. A review of past violations was conducted and a violation notice was completed regarding the one repeat violation and other violations found during today's inspection. The preschool classes went outside to play on the playground, the toddlers were eating lunch and the infants were eating lunch and napping. Lunch, table sanitizing, hand washing and diaper changing was also observed. There are 31 children in care today with five teaching staff and three office staff and one cook present. The office staff go into the classrooms when needed. Should you have any questions please do not hesitate to call. Inspection began: 10:40 am and ended 2:30 pm approximately.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation during the monitoring inspection the center failed to maintain some equipment in a safe manner. Evidence: 1. In the four year old room the rest mats are torn exposing the inside padding.

Plan of Correction: The rest mats will be replaced.

Standard #: 22VAC40-185-340-F
Description: Based on observation during the monitoring inspection on 1/26/17 the staff failed to keep children in actual sight and sound supervision. Evidence: 1. While observing the pre-school rooms on the playground the LI witnessed the classes lining up at separate doors to go back into their classes. 2. Staff # 3 counted her children and called for them to come and line up. Staff # 3 counted again and then took the children inside. As the door was closing the the LI heard the the staff person state "oh no wait", the door closed and then opened again. Staff #3 came out the door leaving six children, ranging in age of 3- 5 years, inside the class room out of sight and sound supervision. This incident lasted approximately one minute.

Plan of Correction: Staff will be reminded that they are to always have children in sight and sound supervision. The existing procedures will be reviewed with the staff.

Standard #: 22VAC40-185-500-A
Description: Based on observation of the center during the monitoring inspection the center failed to wash hands and dispose of diapers in a manner according to standards. Evidence: 1. Staff #4 changed a diaper but did not dispose of the diaper in a foot operated container. 2. Staff #4 failed to wash the child's hands following lunch and diapering.

Plan of Correction: 1. A foot operated trash can was placed in the infant room to be used by the staff. 2. Staff will be reminded to wash children's hands following meals and snacks and diapering.
