Comments:
An unannounced monitoring inspection was conducted with the center's fiscal officer/HR personnel beginning at 8:40 AM and ending at 9:24 AM to review the board members' records. Four corporate officers' records and five board members records were reviewed during the inspection. Violations:
Standard #: 22VAC40-191-40-D-1-C
Description: Based on a review of four corporate officers' records, the center did not obtain updated background checks every three years as required. Evidence: Corporate officer A and B's most recent criminal background checks were documented as September 10, 2012. Corporate officer A and B?s most recent central registry check was documented as September 19, 2012.

Plan of Correction: The center was in the process of requesting updated background checks for board members prior to the inspection.
