Comments:
An unannounced monitoring inspection was completed on January 25, 2017. The purpose of this visit was to measure the facility?s compliance with most but not all of the licensing standards. The following ratios were observed: Preschool-1 staff/9 children;Toddlers-2 staff/10 children.
The toddlers were observed reviewing colors and shapes using manipulatives. The preschool children were completing an activity centered around the number 8. The children were served a lunch of chicken nuggets, baked beans, mandarin oranges, peas, celery and milk. The facility administers medication. Medication and accompanying authorizations were reviewed. MAT certificates were also reviewed. The last evacuation drill was conducted 12/19/16. The most recent shelter in place drill was conducted 12/19/16. Four children?s records and two staff records were reviewed.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on a review of a sample of children's records, the facility did not ensure that immunization records were obtained prior to a child's attendance at the facility. EVIDENCE: The records of child #2, start date 1/23/17, and child #4, start date 1/24/17, did not contain immunization records.

Plan of Correction: We will ensure that parents submit the immunization records prior to the first day they are enrolled at the center. I will contact the parents to request the submission of the immunization records that are due.

Standard #: 22VAC40-185-210-A
Description: Based on a review of a sample of staff records, the facility did not ensure that a staff person was qualified for her position. EVIDENCE: Staff person # 1 did not meet the qualifications required for program leader.

Plan of Correction: We will ensure that the staff person gets the 24 hours of training required, in the next 30 days, in order to qualify for the position.

Standard #: 22VAC40-185-510-E
Description: Based on a review of medication at the facility, the facility did not ensure that a child's medication was labeled with the child's name. EVIDENCE: The over-the-counter medication of child# 1 was not labeled with the child's name.

Plan of Correction: We will ensure that student's names are placed on the medications that are brought from home. Corrected on site.

Standard #: 22VAC40-185-540-C
Description: Based on a review of the transportation first aid kit, the facility did not ensure that the kit contained all of the required items. EVIDENCE: The first aid kit on the van was missing scissors, triangular bandages and a thermometer.

Plan of Correction: Corrected on site.

Standard #: 22VAC40-185-580-B
Description: Based on an inspection of the facility's van, the facility did not ensure that the required documents were in the van. EVIDENCE: The van did not contain the number for the regional poison control center, 911, or a document containing potential shelters, nearest hospitals, evacuation routes or routes frequently driven by staff for center business.

Plan of Correction: All information will be kept on van.
