Comments:
The licensing inspector conducted an unannounced monitoring inspection Friday, January 27, 2017 from 11:30am to 3:10pm. The children were observed resting on cots for rest time. Lunch consisted of chicken with gravy, bread, mixed vegetables, apples, and milk. The lunch menu was posted. All classrooms and playgrounds were inspected today as well as the facility?s mini buses. Three children?s records and one employee record were reviewed during this inspection. Medication is/is not being administered and was reviewed. The center administers medication; however they did not have any being given at this time. There were no injury reports to review at this time. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 12/14/2016 Shelter-in-place drills: 2/24/2016 & 12/15/2016 Last fire inspection: 01/20/2017 Last health inspection: 11/07/2016 Total enrollment today: 39 Today, the following child to staff ratios were observed: Infants-2:1 Older Infants/ Toddlers ?3:1 Preschool (two -five years old, rest time ratio) ?34:2 School Age ? not present The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-70-A
Description: Based on a review of staff records, the center did not ensure that each staff record contained documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering. EVIDENCE: The record of Staff #1, whose date of hire was 10/25/2016, contained documentation of one reference check.

Plan of Correction: Will get other reference checks on Monday. Previous employer was contacted prior to hiring, but not documented because they did not provide any information. Will make note of the dates when contacted previous employer.

Standard #: 22VAC40-185-270-A
Description: Based on observation on 01/27/2017, the center did not ensure that all areas of the center were maintained on a safe condition. EVIDENCE: The second step from the bottom leading to the Preschool trailer is loose and wobbly, causing a tripping hazard.

Plan of Correction: Will get handy man in on Monday or Tuesday to repair step.

Standard #: 22VAC40-185-350-D
Description: Based on observation on 01/27/2017, the center did not ensure that during the designated rest period the ratio of staff to children is double the number of children to each staff for children in the toddler through preschool classes only. EVIDENCE: The ratio in the combined grouping of the older and young infant rooms for rest time was 5 infants to one staff. The ratio in the infant classrooms can not be doubled during rest time.

Plan of Correction: Brought staff in to cover ratio during inspection. Will move older infants into infant room or have a back up person in the room.
