Comments:
A complaint investigation was conducted on 12/6/2016 beginning at 6:33 a.m. and ending at 8:10 a.m. regarding supervision. Classrooms were observed. Documentation and records were reviewed. Interviews were conducted. The exit interview was conducted on 1/26/2017 at 10:30 a.m. The complaint regarding supervision: staff to children ratios was determined to be valid. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it to the office by January 31, 2017. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Description: Based on interviews and observation, the center failed to maintain the required staff to children ratio. Evidence: In the infant room, Staff B was observed alone with six infants ranging in age from four months to twelve months. The required ratio for infants is one staff member per four infants. Interviews revealed staff to children ratios had not been met for at least two months during the opening hours of 6:00 a.m. to 8:00 a.m.

Plan of Correction: Staff member schedules have been changed to meet the demand in the morning. Both staff A and B works from 5:30 to 2:30, staff C comes in at 6 and staff D comes at 6:30am. These schedules have been working very well .

Description: Based on interviews and observation, it was determined the center failed to maintain the required staff to children ratio for two year old children. Evidence: Beginning at 6:33 a.m., in the opening room (the school age classroom) Staff A was alone with eighteen children, ranging in age from two years to nine years old. At approximately 7:01 a.m., Staff A was alone with a classroom of twenty-five children ranging in age from two years to nine years old. The required ratio for two-year olds is one staff member per eight children. A second staff, Staff C, arrived at 7:04 a.m. and transitioned nine children ranging in age from two years to four years of age to the three year old classroom. Two additional children arrived at 7:09 a.m. for a total number of eleven children with Staff C with a required staff to children ratio of one staff to eight children. Interviews revealed staff to children ratios had not been met for at least two months during the opening hours of 6:00 a.m. to 8:00 a.m.
