Standard #: 22VAC40-185-40-D Description: During today's inspection, the license and Violation Notice were not posted in a place conspicuous to the public per the Code of Virginia. Evidence: Staff reported that the license had not yet been put up since the room was painted.. The Violation Notice was found on a clipboard in the office where it was no conspicuous. Plan of Correction: They will be posted.
Standard #: 22VAC40-185-60-A Description: During a review of children's records today, some information was found to be missing. Evidence: There was incomplete emergency contact information for Child # 3. There was no physical available for Child # 3 who began care over 30 days ago, and no physical exam and immunization record available for Child # 2, who began care 11/15. Plan of Correction: One parent was contacted during the inspection and will fax the physical exam and another parent will be contacted later.
Standard #: 22VAC40-185-270-A Description: During today's inspection, some areas and equipment of the center were observed to not be maintained in a clean, safe, operable condition. Evidence: 1. In the infant/toddler room, fitted sheets that were covering two cribs were observed to have holes in the linen. A third crib had a sheet that was so tight, the mattress was curled under and one corner was not covered by the sheet. In the two year old room, two vents were observed to be very dirty, and one had uneven vent openings. 2. In the back toddler room, peeling "snow" paint flakes were observed on the windowsill where they were flaking off in pieces. There was also a wall protector opposite the doorknob that had crumbling soft rubber coming off it, which could present a choking hazard in that room. Outdoors: 3. There was a peeling surface observed at the edge of the preschool playground slide and a broken piece of wood at child height in the gate surrounding the air conditioning unit. Some splintering areas were also visible along the door frame of the outdoor closet. Plan of Correction: New crib sheets (green) were purchased and the sheets with holes will be removed today. The sheet that was too small will also be removed and replaced. The peeling "snow" will be removed by the teacher today. Vents will be cleaned or replaced. A facilities manager was present today and the doorknob protector was pointed out; as well as areas outdoors. He took photos of areas.
Standard #: 22VAC40-185-430-A Description: During today's inspection, an infant was observed to be lying on the floor near a toy that was not age and stage appropriate. Evidence: A small stuffed toy observed today to be accessible in the infant room had a label on it that read "Not for use in cribs or playpens." A 10 inch (approx) long pink ribbon was attached to it at one end. Staff reported they did not know where it came from. Plan of Correction: Removed during inspection.
Standard #: 22VAC40-191-40-D-3-D Description: A review of staff records today found that one staff did not have results of search of the central registry in the last three years. Evidence: Staff # 3, who began work 3/13, did not have results of the Search of the Central Registry as required. A form had been mailed to DSS with a stamped date of 4/13 but there was no back page of the search with the results on it. The center sent out another form signed on 12/6/16, but that one was returned due to an administrative problem. Plan of Correction: This teacher came from another center, we did not do the original Search of the Registry. A new one will be sent for as soon as possible.
