- Technical Assistance:
Technical assistance was given regarding the location of the leakproof receptacle in the diapering area for soiled linens. Technical assistance was given regarding the clear labeling of a child's name on medication.
An unannounced monitoring inspection was conducted on February 1, 2017 from approximately from 8:15 a.m. until 11:35 a.m.. At the time of entrance there were 6 children and 2 staff members were present with children. There were additional staff on site for administrative purposes. The sample size consisted of 2 children's records and 3 staff records. Two medications were reviewed. Children were observed preparing for and eating breakfast, reading and playing in centers. Areas of non-compliance are identified on the violation notice. The results of the inspection were reviewed with the assistant center designee during the exit interview on this date.
Standard #: 22VAC40-185-40-I Description: Based on a documentation review, the licensee did not ensure a written procedures for injury prevention was developed. Evidence: During the inspection on February 1, 2017, there was not a written injury prevention plan available for review. Plan of Correction: The assistant site manager will work with the Deputy Director to get a prevention plan written.
Standard #: 22VAC40-185-70-A Description: Based on a record review and staff interviews, the licensee did not ensure there was a complete record for all employees. Evidence: During the inspection on February 1, 2017, the following records were determined to be incomplete; 1) The record for staff #1 did not include documentation indicating they have the education/ certifications / training required for the program director position. The Education and Disabilities Coordinator (staff #4) is the supervisor for staff #1 and they confirmed staff #1 is designated as the centers program director. 2) The records for staff #2 and staff #3 do not include documentation indicating they have the education/ certifications / training required for the program leader position. Staff #4 confirmed staff #2 and staff #3 are designated as program leaders. 3) The record for staff #3 did not include documentation indicating two or more references as to character and reputation as well as competency were checked before employment. Plan of Correction: The assistant site manager will work with the site manager to get the documents.
Standard #: 22VAC40-185-570-E Description: Based on observation and staff interviews, the licensee did not ensure prepared infant formula was refrigerated, dated and labeled with the child's name. Evidence: During the inspection on February 1, 2017, one bottle was observed without a date. Plan of Correction: The assistant site manager will talk with the staff to make they are aware of the requirements
