- Technical Assistance:
1. Reviewed retention background check requirements with new human resources personnel.
Conducted an unannounced monitoring inspection 10:20am-12:05pm. Observed 9 children in the preschool with 1 direct-care staff. Ratios and supervision were in compliance. Interactions between staff and children were positive, and behavior guidance was redirective in nature. Children were having story time, washing hands, eating snack, and playing with puzzles during the inspection. Prior to the inspection they had made letter "P" artwork with popcorn. Snack served: rice chex cereal, bananas, blueberries, cranberry-apple juice, and water. The classroom was found to be clean, and well supplied with toys and equipment for the children. No medications reported to be on-site. First Aid supplies were in order. Required documents were posted. Record keeping violations found. Standard #: 22VAC40-191-60-B Description: Based on records reviewed, it was determined that 1 of 3 staff records did not include documentation of a completed Sworn Disclosure Statement (SDS). Evidence: There was not a completed SDS on-file for Staff #8 who was hired 1/9/17. Plan of Correction: To be signed and filed 1/9/17
