An unannounced monitoring inspection was conducted today, February 6, 2017 from 2:45pm until 3:40pm. The inspector observed 52 children directly supervised by 4 staff members. Ratios and supervision were in compliance. A sample of 4 children's records were reviewed. The physical plant, medications, emergency supplies and procedures were also reviewed. The children were engaged in various activities, to include: playing outdoors, participating in an ice cream social transitions. Areas of non-compliance are identified in this report. Description: Based on record review, one child's file was incomplete. Evidence: Child #3 has one of two required name, address, and phone numbers of a designated person to call in an emergency if a parent cannot be reached.

Plan of Correction: We will contact the parent tomorrow to get the second emergency contact.
