Discussion was held on the topics of current enrollment and current staffing.

Comments:
The following inspection took place between the hours of 10:30 am through 1 pm. Today, I met with the Staff of the Montessori School located in the town of Lovettsville, Virginia. At the start of this unannounced renewal inspection there were a total of 20 children with 3 staff; good ratios in both rooms. A total of 5 children?s files and 4 staff files were reviewed. The children were engaged in the following: learning centers within each room, large motor skills on the center playground, bathroom breaks, hand washing, piano lessons, song, games, lunch, and other organized activities. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review, 2 of 5 children's files reviewed indicated that the center did not maintain and keep within each enrolled child's record all of the required information and documentation. Evidence - 1. On the date of inspection (02/09/2017), health information as required by 22 VAC40-185-130 through 22 VAC40-185-150 (to include a physical and immunization page) for Child #2 was not available for review. Based on the information on file, the start date for Child #2 was listed as - 08/30/2016. 2. On the date of inspection, the proof of identification for Child #3 was not available for review. Based on the information on file, the start date for Child #3 was also listed as - 08/30/2016.

Plan of Correction: The Parents of Child #2, and Child #3 will be contacted and both the applicable health information and applicable proof of identification shall be obtained.

Standard #: 22VAC40-185-280-C
Description: Based on observation, insecticides were stored in an area used for food storage. Evidence - 1. On the date of inspection, within a hallway closet, a small green basket kept on the upper most shelf was observed filled with multiple spray-cans of inspect repellent. 2. Within this same closet was observed various stored boxes and containers of dry-good foods to be used at a later time. 3. The spay-cans showed no signs of leakage, but remained intact.

Plan of Correction: The basket shall be moved to a different area.
