Comments:
An unannounced renewal inspection was conducted on Thursday, February 9, 2017, from 10:00am until 4:00pm. The following staff to child ratios were observed:
Infants: ten infants with three staff: infants were observed having floor play with staff and during diapering procedures.
Toddlers: ten toddlers with two staff: children were observed completing puzzles and preparing for lunch and naptime.
Room 2: eight two year old children with two staff- children were preparing to bowl in the classroom.
Room 3: seven two year old children with one staff - children were playing in home living and preparing for small group art projects.
Room 4: seven two year old children with one staff- the children were playing in the transportation center.
Room 6: seven three year old children with one staff: the children were tracing their names, building with legos and making Valentine's gifts.
Room 7: nine three year old children with one staff: the children were making Valentine's gifts, playing with cars, and at the sensory table that was filled with various red items.
Room 8: seven four year old children with one staff: the children were designing Valentine's Day bags and playing in dramatic play wearing various outfits.
Room 9: eight four year old children with one staff: the children were building with legos, matching letter to picture puzzles and completing a math/science activity.
Lunch: cheeseburgers, applesauce, cucumber slices and milk
Five staff are currently certified in Medication Administration; fifteen of twenty-five staff are currently certified in CPR and first aid; nine staff are currently certified in Daily Health Observation.
Three staff records and five children's records were inspected. Five medications were inspected.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on inspection of staff records, the center did not ensure all staff submitted documentation of tuberculosis screening prior to employment or within 21 days after employment. Evidence: Out of three staff records inspected, Staff #1 started 11/21/16 and did not provide documentation of TB until 1/9/17.

Plan of Correction: Per Center: "This was corrected when we did a self audit. Future TB screenings will be done within 21 days of employment."

Standard #: 22VAC40-185-270-A
Description: Based on inspection of the facility, the center did not ensure all areas were maintained in a safe condition. Evidence: In Classroom #6 and Classroom #9 a floor activity rug was present that would slide across the floor when stepped on posing a tripping hazard.

Plan of Correction: Per Center: "Transportation center rugs were removed from the classroom until nonskid adhesive would be applied."

Standard #: 22VAC40-185-280-B
Description: Based on inspection of the facility, the center did not ensure all hazardous substances such as cleaning materials were kept in a locked place. Evidence: Out of nine classrooms inspected, hazardous cleaning materials were found in an unlocked cabinet in Classroom #4. These items were located in high cabinets that were inaccessible to children, however, they were not locked.

Plan of Correction: Per Center: "The materials were used for an art activity and have now been removed from the classroom."

Standard #: 22VAC40-185-500-B
Description: Based on observations, the center did not ensure required diapering procedures were followed. Evidence: In the toddler classroom, the licensing inspector observed diapering. The diapering surface must be cleaned with soap and water and then sanitized after each child is changed. The staff person did not clean the diapering surface with soap and water between each child's diaper being changed, however, the surface was sanitized between each diapering.

Plan of Correction: Per Center: "Staff were spoken to about this while the inspector was here."

Standard #: 22VAC40-185-510-D
Description: Based on inspection of children's medications present at the facility, the center did not ensure all authorizations were present. Evidence: Out of five medications inspected, the medication authorization for Child #4, could not be located.

Plan of Correction: Per Center: "The parent will be given a new authorization form to complete. The medication will be returned to the parent until the new authorization form is completed."

Standard #: 22VAC40-185-570-A
Description: Based on observations, the center did not ensure the safety belts for individual seats in the feeding table were secured. Evidence: The toddler children were placed in the individual seats of the feeding table without staff securing each safety belt.

Plan of Correction: Per Center: "Staff will be reminded to always fasten the safety belts when children are placed in the feeding table."

Standard #: 22VAC40-191-40-D-1-C
Description: Based on inspection of staff records, the center did not ensure all background clearances were completed before three years since the dates of the last sworn statement, most recent central registry finding and the most recent criminal history record check report. Evidence: Out of three staff records inspected Staff #3 criminal history record expired 12/16/16 and the central registry finding expired 12/1/16.

Plan of Correction: Per Center: "During a routine self audit this was caught and the forms have been mailed out."
