Comments:
An unannounced monitoring inspection was conducted today, February 13, 2017 to determine the center's compliance with licensing standards. Upon arrival, the following ratios were observed:

Head Start (2 yr old) room: 2 staff/ 10 children
Head Start (3yr old) room: 2 staff/ 10 children
Three year old room: 1 staff/ 8 children
Four-Five year old room: 1 staff/ 9 children
School age room 1: 2 staff/ 13 children
School Age room 2: 1 staff/ 8 children
School Age room 3: 1 staff/ 10 children
School age room 4: 1 staff/ 5 children

Children were observed eating snack, using the restroom, coloring pictures, and completing educational worksheets. Today's snack consisted of Sloppy Joe, carrots, an orange, a roll, and milk. Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. All classrooms were inspected to include main hallways and children's restrooms. The outdoor playground was inspected with the new resilient surfacing added.

The center had the following postings displayed near the entrance and in children's classrooms. The center's first aid kit and emergency supplies were inspected with required supplies found. Five children's and two staff records were inspected with the majority of the required information found captured; see attached notice. The following was also inspected: medication & authorizations, annual health & fire inspections, fire drill log, parent agreements, and other pertinent information.

Standard #: 22VAC40-185-60-A
Description: Based on the inspection of administrative records, the required information was not in one child's record.

Evidence:
During the inspection of five children's records, child #2 did not have the street address for both emergency contacts.

Plan of Correction: Per director: I will have the parent to update the emergency contacts address during pickup.

Standard #: 22VAC40-185-550-B
Description: Based on the inspection of the center's emergency preparedness plan the plan did not contain the required route information.

Evidence:
During the inspection of the center's emergency preparedness, it was discovered the plan did not contain primary and secondary routes of egress.

Plan of Correction: Per director: I will have the plan updated.
